GENERAL SUMMARY:
The Integrated Social Worker is a member of the clinical healthcare team who will assist with managing the overall health
of patients.
The Integrated Social Worker will help improve recognition, treatment, and management of social health, mental health, psychosocial/behavioral problems, and medical conditions.
This includes providing resource coordination for identified patients, identifying the psychological, behavioral, emotional, and social factors essential to the prevention, treatment, or management of social health problems as it relates to a patient’s physical health.
MINIMUM QUALIFICATIONS: 2 years of experience as a social worker in a primary care or acute care setting and community mental 2 years of experience with unrestricted Ohio License to practice as LSW Excellent interpersonal and written communication skills required health center preferred Medicare and Medicaid provider numbers WORKING CONDITIONS: Ability to deal with upset or disgruntle patients in order to deescalate the situation.
Average working hours with possible non-traditional hours.
Carry up to 20 lbs occasionally.
Lifting up to 50 lbs with or without an accommodation.
Potential for exposure to blood and body fluids.
Standing 1-2 hours at a time Walking approx.
30 minutes per hour throughout the day.
Withstanding pressure and deals with emergency situations as needed.

SPECIFIC DUTIES AND RESPONSIBILITIES: Advocate for and empower patients to maintain a level of control over their health and wellness.
Appropriately document assessments and interventions in the medical record within 48 hours of service.
Assist the patient in obtaining, processing, and understanding the necessary health information and services needed to make appropriate health decisions.
Collaborate with members of the healthcare team, the patient/family, and community liaisons to address patients’ psychosocial and socioeconomics.
Collaborate with the healthcare team to move the patient towards self-management.
Formulation of social interventions appropriate to the primary care setting and assist with the implementation of PCP treatment plans.
Identify and address social barriers impeding the health of patients.
Integrated LSW will refer to LISW for diagnosis and treatment of ongoing mental health issues that require more specialize treatment Maintain current in the field by attending seminars, continuing education courses, workshops, etc.
Maintain knowledge of resources available in the area community.
Manage a specific caseload of patients referred by clinic staff and the Social Determinants of Health screening tool.
Monitor the outcome of care using patient-reported outcome measures.
Practice appropriate and timely scheduling and billing practices.
Provide and maintain patient educational materials for patients and staff.
Provide close monitoring of patients through follow up contacts and coordinate services with other providers as necessary.
Provide crisis intervention; assess and coordinate protective service issues.
Provide culturally sensitive care, patient-centered, and whole-person care.
Provide social work interventions to remove psychosocial barriers to health and wellness based on the Social Determinants of Health screening tool.
Share knowledge with other team members and patients, both formally and informally, through the use of the EHR system.
Teach and provide education to promote understanding and skill-building to enhance psychological, social, and physical health.
Triaging and referring patients to social services when appropriate.
