*Registered Nurse (RN):* The RN Case Manager provides services to clients in their homes, following a care plan developed through the collaboration of the physician, agency, client, family and other members of the healthcare team.
*Qualifications:* · Possesses a current license to practice as a registered nurse in Ohio.
*Responsibilities:* The staff nurse/ case manager is a registered nurse who administers skilled nursing care to clients and teaches the client, their families and other members of the nursing team regarding the care of the client.
Each staff nurse performs nursing services for clients in accordance with the physician’s orders.
*Qualifications:* * Graduate of an approved school of professional nursing.
* Possesses a current license to practice as a registered nurse in Ohio.
*Responsibilities:* * Conducts the admission of the client to the Agency by: * Obtaining a medical history, particularly as it relates to the present condition, from the client and/ or family members.
* Conducting a physical examination of the client, including vital signs, physical assessment, mental status, functional ability, current medications, diet, etc.
* Evaluating the client, family members and home situation to determine what health teaching may be required.
* Evaluating what family assistance will be available to the client.
* Explaining what nursing or other services will be made available to the client by the Agency.
* Developing along with the client and/ or family the nursing care plan, goals and discharge planning.
* Provides skilled nursing care as outlined in the nursing care plan to include the following: * Nursing services, treatments and preventative procedures requiring specialized skill and as ordered by the physician.
* Initiating preventative and rehabilitative nursing procedures as appropriate for the client’s care and safety.
* Observing signs and symptoms and reporting to the physician any changes in the client’s physical or emotional condition.
* Teaching, supervising and counseling the client and caregivers regarding the nursing care needs and other related problems of the client at home.
* Is responsible for the supervision of care being given by the home health aide at a minimum of every two weeks.
* Is responsible for obtaining physician orders for all care given, contacting physician with any changes in patient condition that might warrant changes in physician orders for treatment and updating physicians regarding patient status relating to progress toward meeting discharge goals.
* Is responsible for properly documenting all client care and education and submitting clinical notes weekly to the Agency.
* Making revisions in the nursing care plan as needed, evaluating client care and progress and closing charts of discharged clients.
* Assists client and/ or family members by making community referrals as may be necessary.
* Submits accurate time sheets to coincide with clinical progress notes on a weekly basis.
* Seeks counsel of the Director of Nursing for clarification of care plans as needed.
* Participates in client care conferences and staff meetings as necessary.
* Prepares the home health care plan.
* Keeps the Director of Nursing advised of client’s conditions, progress, problems, complaints and any unusual occurrences.
* Must maintain current licensure and continuing education requirements and provide evidence of such to the Agency.
*Organizational Relationship:* * Reports directly to the Director of Nursing.
*Functional Abilities:* · Must be able to read 12 point type or larger.
· Must be able to communicate verbally and in writing in a manner understood by most persons.
· Must be able to stoop, bend and be able to lift and transfer clients.
· Must be able to travel to client’s residences.
*Special requirements:* · Must have a reliable car, car insurance and a valid driver’s license.
· Must be able to successfully pass a background check.
